So, what is a certificate of insurance anyway?

A Certificate of Insurance (COI) is a simple document that shows your ministry’s active insurance coverage. Venues, landlords, or partner organizations often ask for one before you can rent space, sign a lease, or host an event. This form helps us prepare your certificate quickly so you can stay focused on ministry.

Once you complete this form, your request goes directly to your ChurchWest account manager. They’ll review the details and prepare your certificate. When it’s ready, they’ll send it to you and any other organizations that need a copy. Most certificates are turned around within one business day.

What kind of coverage does your certificate need to show?

The Certificate Holder

The Certificate Holder is the organization that has asked you to provide proof of insurance. This might be a landlord, a city, a school, a venue, or another partner. Enter their information below so we can make sure the certificate is prepared correctly and sent to the right people.

Do they need to be added to your policy?

Sometimes a certificate holder doesn’t just want proof of insurance — they want to be formally added to your policy in a special role. This gives them certain rights, like receiving notices if your coverage changes, or even being covered under your insurance for specific situations. Only check the box that matches what they’ve explicitly requested in writing.

Your landlord wants to be listed because you rent or lease their space.

A leasing company or lender wants to be paid first if insured property (like equipment) is damaged.

A bank or lender on your ministry's loan wants notice and / or protection if there’s a loss.

Churches and ministries request certificates for lots of different reasons. Sometimes it’s for a specific event (like a youth camp, concert, or outreach), and other times it’s for an ongoing need (like a lease or loan). Some of the questions below assume your request is for an event. If it isn’t, just skip those fields—your account manager will make sure your certificate is prepared correctly.
